Drug giant Pfizer's blockbuster COX-2 inhibitor Bextra was pulled from the market on April 7, 2005. Also that day, the FDA released an "Alert for Healthcare Professionals" warning that Bextra caused "unpredictable," "serious and potentially life-threatening skin reactions (e.g., Toxic Epidermal Necrolysis (TEN), Stevens-Johnson Syndrome (SJS), Erythema Multiforme)" at a rate "significantly greater" than other drugs in its same class of medications (COX-2 inhibitors). Combined with the fact that Bextra had not been shown to offer any unique advantages over other available drugs, the FDA concluded that that the risks o Bextra outweighed its benefits.

Stevens-Johnson Syndrome (SJS) is a type of allergic reaction known as an "immune-complex-mediated hypersensitivity condition." It is one of three related skin diseases of varying severity linked to the use of Bextra, the two others being the less severe Erythema Multiforme (EM) and the painfully disfiguring and potentially deadly Toxic Epidermal Necrolysis (TEN).

What is Bextra-Induced Erythema Multiforme (EM)?
The most mild Bextra skin reaction related to Stevens-Johnson Syndrome (SJS) is Erythema Multiforme (EM). The onset of Erythema Multiforme (EM) usually is characterized by the rapid outbreak of multiple skin lesions that may appear as red dots, sometimes resembling a skin rash from poison ivy. Over time, some of the spots may blister and leave permanent scarring after they heal. This mild form of Stevens-Johnson Syndrome (SJS) is called Erythema Multiforme Minor and usually resolves in 6-10 weeks or less.

True Stevens-Johnson Syndrome (SJS) is a very serious and potentially life-threatening Bextra skin reaction. The onset of Stevens-Johnson Syndrome (SJS), also sometimes referred to as Erythema Multiforme Major, usually is characterized by the appearance of round skin lesions surrounded by concentric red and pale rings of inflamed tissue. This red skin rash of circular "target" or "bull's-eye" lesions develops in clusters, usually with swelling and blistering, typically first on the hands and feet, then often spreading to the skin on the arms, legs, and face, and to the mucous membranes of the lips, gums, tongue, eyes, and genitals. Sores and "stickiness" in the eyes, genitals, and other mucous membranes also may occur. It is not unusual for lesions to spread through the mucous membranes of the throat into the lungs, stomach, intestines, and other major organs.

The Most Severe Progression of Stevens-Johnson Syndrome (SJS)
In the most severe cases of Stevens-Johnson Syndrome (SJS) / Erythema Multiforme Major, groups of the inflamed skin and mucous membrane lesions coalesce into gooey blisters of dead tissue that fall away from living tissue leaving it unprotected and open to infection. This most severe form of Stevens-Johnson Syndrome (SJS) / Erythema Multiforme Major is called Toxic Epidermal Necrolysis (TEN) and often is fatal.

In addition to the skin conditions described above, the onset of Stevens-Johnson Syndrome (SJS) / Erythema Multiforme Major also commonly is associated with a bad cough or a sore throat from a nonspecific upper respiratory infection. Other early symptoms of Stevens-Johnson Syndrome (SJS) / Erythema Multiforme Major may include whole body flu-like symptoms including fever, headache, chills, diarrhea.
